Card Benefits

Free issuance of the Deendayal Antyodaya Upchar Yojana Card.

Enables access to government-funded medical treatment schemes.

Inclusive coverage for persons with disabilities belonging to BPL families.

No administrative fee is charged by the department.

Mode of Disbursement

Physical card issued through designated health officers or Lok Seva Kendra.

Card issued within 7 working days from receipt of complete application.

One-time card issuance per eligible family.

The applicant should be a resident of Madhya Pradesh.

The applicant should belong to a below-poverty-line family.

The applicant should be a resident of the same district or area where the application is submitted.

The applicant should be the head of the family.

The applicant’s family may include persons with disabilities, without exclusion.

Step 1: The interested applicant downloads the application form (Annexure 1) from the official website.

Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).

Step 3: Submit the duly filled and signed application form along with the documents to the respective designated officer or Lok Seva Kendra.
