The applicant must not hold any position under Government/Semi Government Authorities/Boards.

The applicant must not be a member of Panchayati Raj Institutions.

*Preference should be given to widow/aged/unmarried women/physically handicapped/Below Poverty Line (BPL) families.

Certificate From The Hospital/Nursing Home Where Undergoing Treatment (PDF Maximum 1MB). *This document serves as the Medical Certificate from the Government Doctor not below the rank of Sub-Divisional Medical Officer or from the Doctor of the Nursing Home/Hospital wherein the patient is undergoing treatment, confirming the disease of the patient.
