Application Process:

Step 1: The interested applicant can download the application form from the official website or collect it from the Directorate Office near the 2nd MR Gate.

Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).

Step 3: Submit the duly filled and signed application form along with the documents to the District Social Welfare Officer

Step 4: Request a receipt or acknowledgment from the Concerned District Social Welfare Officer to whom the application has been submitted. Ensure that the receipt contains essential details such as the date and time of submission, and a unique identification number (if applicable).

Passport-size Photo.

Aadhar Card (as proof of Identity).

Certification that the applicant is not a beneficiary of any existing government schemes for individual beneficiaries (issued by the Panchayat or Municipality or District Council).

Unemployment Certificate/Income Certificate (issued by Gazette Officer).

Matriculation Certificate or Birth Certificate (as proof of age).

Residence Certificate (issued by a Gazette Officer, with seal and signature).

Bank Passbook.
