Accidental Death

₹50,000/- per student.

Accidental Permanent Disability

In case of loss of two eyes or both limbs or arm/leg due to accident: ₹50,000/-.

In case of loss of one eye and one limb due to accident: ₹50,000/-.

In case of loss of one eye or one limb due to accident: ₹25,000/-.

Conditions

The incident must be unexpected, sudden, and unintentional, and show visible signs of external violence.

The claim application must be submitted to the Nodal Officer within ninety (90) days from the date of the accident or death.

The benefit amount must not be paid for more than one category if the beneficiary is covered under multiple categories of the overall scheme.

For the Student Beneficiary

The beneficiary must be a student studying in all Primary, Secondary, and Higher Secondary schools of Gujarat.

For the Applicant/Claimant

The applicant must be the successor or the disabled beneficiary themselves in case of permanent disability.

The applicant must appear in person with the original identity card.

The applicant must submit the written application in the prescribed format to the local Nodal/Competent Officer.

The application must be submitted within ninety (90) days from the date of death or accident.

Offline

Step 1: The applicant must obtain the necessary application forms and annexures through the Principal of the concerned school where the student was studying.

Step 2: The applicant must fully and comprehensively fill out the prescribed application form and gather all mandatory supporting documents to substantiate the claim. The application must be accompanied by the original Identity Card.

Step 3: The applicant (the heir in case of death, or the beneficiary in case of permanent disability) must submit the completed application along with all required proofs to the local Nodal/Competent Officer of the controlling office of the relevant department. This submission must be done in person (not via post) within ninety (90) days from the date of the accident or death.

Step 4: The Nodal Officer is responsible for fully checking and verifying the application and must forward the application, along with a certification in the prescribed format, to the Office of the Director of Insurance or the Insurance Company. This forwarding must occur within thirty (30) days of the Nodal Officer receiving the application.



Post-Application Processes

Step 1: Resolution by Insurer

The Insurance Company or the State Insurance Fund must decide on the claim approval or rejection within sixty (60) days of receiving the application with complete and satisfactory information/documents.

Step 2: Claim Closure Timeline

If the applicant fails to provide all requested documentation and fulfillments within the stipulated time, the claim will be closed automatically. The claim will also be permanently closed twelve months after the date of the accident/death, and no reconsideration request will be accepted thereafter.



*If a claim is rejected, the beneficiary or their heir must first submit a representation to the designated Review Committee within thirty (30) days.

*Legal action against the Review Committee's decision can only be pursued after filing the review, and the legal/quasi-judicial/judicial jurisdiction is exclusively in Gandhinagar.

*Claim inquiries or representations must be made only by the applicant or beneficiary themselves, excluding any third parties like agents or lawyers.

Documents Required to Establish Student Status

Certificate From the Principal of the School Stating That the Student Is Studying In the Specific Class/Standard.

Original Certified Copy of the School Leaving Certificate, Where the Date of Leaving Must Be On Or After the Date of Death Or Injury.

Certified Copy of the Identity Card (If Provided).

Mandatory Claim Documents (General)

Proof of Identity of the Applicant.

Proof of Identity of the Deceased.

Certified Copy of the First Information Report (FIR).

Certified Copy of the Police Panchnama And Inquest Panchnama.

Accidental Death Register.

Certified Copy of the Death Certificate.

Certified Copy of the Ration Card.

Heirship Certificate (Pedhinamu) As Per Instruction Number 9.

Indemnity Bond As Per Annexure (Revised).

Advance Receipt of Money As Per Annexure-3 (With Revenue Stamp).

Passport Size Photograph.

Bank Passbook / Cancelled Cheque.

Details And Documents Related To Hospital Treatment After the Accident, Such As Discharge Summary.

Birth Certificate (Or Election Identity Card, only if the deceased/injured is illiterate and the Birth Certificate is unavailable).

Mandatory Claim Documents (Accidental Death Specific)

Certified Copy of the Charge Sheet / Detailed Report Under Section 173 of the Code of Criminal Procedure (CRPC).

Dying Declaration.

Certified Copy of the Final Police Investigation Report Presented To the Sub-Divisional Magistrate (SDM) With the SDM's Order.

Certified Copy of the Postmortem Report.

Certified Copy of the Forensic Science Laboratory (FSL) Report (If Applicable, where chemical analysis of organs was sent).

Certificate From the Doctor Who Performed the Postmortem Stating the Cause of Death.

Mandatory Claim Documents (Permanent Total Disability Specific)

Final Assessment Certificate From the Medical Board / Civil Surgeon.

Postcard Size Photograph Showing Disability.

Collector's Accident Death Certificate (Where Postmortem Was Not Done In Extraordinary Circumstances, Police Papers Are Mandatory).
